KEBIJAKAN PRIVASI

Yayasan Sehatkan Negeri Indonesia adalah suatu yayasan yang didirikan berdasarkan hukum Indonesia dan berkantor di Lantai 1, Jl. KH Mas Mansyur No.19, Karet Tengsin, Tanah Abang, Jakarta Pusat, DKI Jakarta, 10250, Indonesia ("Kami"). Dalam penyelenggaraan usaha Kami, Kami berkomitmen untuk melindungi dan menghormati privasi Anda berdasarkan Kebijakan Privasi ini ("Kebijakan").

Dalam Kebijakan ini, "Anda" mencakup siapa saja yang berbelanja di apotek Kami atau yang memberi Data Pribadi kepada Kami untuk tujuan tertentu sebagaimana diatur dalam Kebijakan ini.

"Data Pribadi" berarti data tentang orang perorangan yang teridentifikasi atau tersendiri atau dikombinasi dengan informasi lainnya baik secara langsung maupun tidak langsung melalui sistem elektronik atau non-elektronik.

Kebijakan ini mengatur:

  • tujuan pemrosesan Data Pribadi;
  • jenis dan relevansi Data Pribadi;
  • jangka waktu retensi dokumen yang memuat Data Pribadi;
  • rincian mengenai informasi yang dikumpulkan;
  • jangka waktu pemrosesan Data Pribadi; dan
  • hak Anda.

Pengumpulan Data Pribadi

Anda dapat memberikan Data Pribadi kepada Kami saat Anda:

  • ingin menerima katalog Kami;
  • melakukan pembelian dari Kami baik dengan resep dokter maupun tidak;
  • mendaftar untuk menjadi anggota pelanggan Kami;
  • mendaftar untuk kegiatan promosi atau lainnya yang diselenggarakan oleh Kami;
  • berpartisipasi dalam survei yang diadakan oleh Kami;
  • mengembalikan produk kepada Kami; dan
  • menghubungi perwakilan Kami secara langsung dan memberikan informasi kepada Kami secara tertulis, melalui telepon, media sosial, atau email.

CCTV

Kami menggunakan CCTV dalam apotek Kami untuk tujuan keamanan dan pencegahan kejahatan serta pengawasan operasional kegiatan apotek.

Data rekaman CCTV akan disimpan dalam jangka waktu yang memungkinkan Kami untuk membantu badan pengawas dan lembaga penegak hukum dalam penegakan hukum atau sesuai dengan ketentuan hukum yang berlaku.

Persetujuan Pemberian Data Pribadi

Saat Anda memberikan Data Pribadi Anda kepada Kami maupun pada saat Anda melakukan transaksi pembelian, Anda dianggap menyetujui pemberian Data Pribadi Anda dan telah mengerti Kebijakan ini.

Kami akan memprosesnya sesuai dengan Kebijakan ini. Jika Anda tidak ingin Kami memproses Data Pribadi Anda, Kami harap jangan berikan Data Pribadi Anda kepada Kami maupun melakukan transaksi dengan Kami yang mengakibatkan perlunya pemrosesan Data Pribadi Anda.

Apabila Anda adalah anak atau penyandang disabilitas sesuai dengan peraturan yang berlaku, Kami menganggap Anda telah memperoleh persetujuan dari orang tua atau wali Anda.

Data Pribadi yang Diproses

Data Pribadi yang diproses oleh Kami adalah:

  • nama, alamat, tanggal lahir, nomor kartu identitas, nomor kartu keluarga, dan/atau nomor paspor Anda;
  • alamat pengiriman;
  • nomor telepon (termasuk nomor telepon seluler) dan alamat email Anda;
  • kondisi kesehatan, informasi medis dan obat yang digunakan sebelumnya, sedang atau yang akan digunakan atau dibeli;
  • informasi resep, termasuk informasi dokter Anda (nama, spesialisasi, lokasi dan informasi kontak);
  • informasi keuangan untuk kepentingan pembayaran termasuk kartu kredit atau kartu debit atau informasi pembayaran lainnya;
  • informasi yang Anda berikan saat mengisi formulir di situs web atau saat berhubungan dengan Kami;
  • catatan atau basis data pembelian produk atau transaksi lainnya antara Anda dan Kami; dan
  • data lain yang dianggap sebagai Data Pribadi yang diberikan oleh Anda kepada Kami.

Tujuan Pemrosesan Data Pribadi

  • memberikan layanan yang baik kepada Anda dan memenuhi permintaan Anda;
  • mengirim informasi kegiatan, pemasaran, atau promosi yang diselenggarakan Kami kepada Anda;
  • memproses pembelian, pembayaran atau transaksi lainnya yang diminta oleh Anda termasuk pengiriman pesanan Anda;
  • melakukan analisa statistik untuk pengembangan dan peningkatan kualitas layanan Kami; dan
  • tujuan lainnya sesuai dengan ketentuan yang berlaku.

Keterlibatan Pihak Ketiga

Kami dapat mengungkapkan dan mengirim Data Pribadi Anda kepada:

  • pihak ketiga untuk kepentingan pemrosesan sesuai tujuan;
  • penyedia layanan yang bekerja sama dengan Kami; dan
  • afiliasi Kami.

Akses dan Koreksi

Jika Anda ingin mengakses atau memperbaiki Data Pribadi Anda, Anda dapat melakukannya secara lisan di apotek Kami atau melalui email ke:

Retensi dan Penghapusan

Kami akan berhenti menyimpan atau memproses Data Pribadi Anda apabila tidak lagi relevan dengan tujuan pengumpulan data tersebut atau tidak diperlukan lagi secara hukum.

Transfer Data

Data Pribadi dapat ditransfer ke luar negeri untuk keperluan hosting server. Kami akan memastikan perlindungan yang setara dengan hukum yang berlaku di Indonesia.

Perubahan dan Keamanan

Kami dapat mengubah Kebijakan ini dari waktu ke waktu dan akan memberi notifikasi sebelum berlaku. Kami menerapkan praktik keamanan standar industri, termasuk enkripsi SSL dan proteksi sandi.

Pelanggaran Data

Jika terjadi pelanggaran data, Kami akan memberi tahu Anda sesegera mungkin (maksimal 3 x 24 jam) sesuai hukum yang berlaku.

Tanggung Jawab

Kami tidak bertanggung jawab atas kerugian akibat keadaan kahar, kesalahan, dan/atau kelalaian Anda.

PRIVACY POLICY

Sehatkan Negeri Indonesia Foundation is a foundation established under Indonesian law and having its office at 1st Floor, Jl. KH Mas Mansyur No.19, Karet Tengsin, Tanah Abang, Central Jakarta, DKI Jakarta, 10250, Indonesia ("We" or "Us" or "Our"). In carrying out Our business, We are committed to protecting and respecting Your privacy based on this Privacy Policy ("Policy").

In this Policy, "You" includes anyone who shops at Our pharmacy or who provides Personal Data to Us for certain purposes as set out in this Policy.

"Personal Data" means data about an identified or identifiable individual, either alone or in combination with other information, directly or indirectly, through electronic or non-electronic systems.

This Policy regulates:

  • the purpose of processing the Personal Data;
  • the type and relevance of the Personal Data;
  • the retention period for documents containing the Personal Data;
  • details of the information collected;
  • the processing period for the Personal Data; and
  • Your rights.

Collection of Personal Data

You may provide Personal Data to Us when You:

  • wish to receive Our catalogue;
  • make a purchase from Us whether with or without a prescription;
  • register to become a member of Our customers;
  • sign up for promotions or other activities organized by Us;
  • participate in surveys conducted by Us;
  • return products to Us; and
  • contact Our representative directly and provide information to Us in writing, by telephone, social media, or email.

CCTV

We use CCTV in Our pharmacies for security purposes and crime prevention, and operational monitoring of pharmacy activities.

CCTV recording data will be stored for a period of time that allows us to assist regulatory bodies and law enforcement agencies in enforcing the law or in accordance with applicable laws.

Consent to Provision of Personal Data

When You provide Your Personal Data to Us or when You make a purchase transaction, You are deemed to agree to the provision of Your Personal Data and have understood this Policy.

We will process it in accordance with this Policy. If You do not want Us to process Your Personal Data, We hope you do not provide Your Personal Data to Us or transact with Us which may result in processing of Your Personal Data.

If You are a child or a person with disabilities according to the prevailing regulations, We assume that You have obtained the approval from Your parent(s) or guardian for a child, or the guardian of the person with disabilities, for a person with disabilities which requires an approval from his/her guardian according to the prevailing regulations.

Personal Data Processed

Personal Data processed by Us are:

  • Your name, address, date of birth, identity card number, family card number, and/or passport number;
  • shipping address;
  • Your telephone number (including mobile phone number) and email address;
  • health conditions, medical information and medicines which were used previously, are or will be used or purchased;
  • prescription information, which includes Your doctor information (including name, speciality, location and contact information), and other related information;
  • financial information for payment purposes including credit cards or debit cards or other payment information;
  • information You provide when filling out forms on the website or when contacting Us via telephone, social media, or email;
  • records or databases of product purchases or other transactions between You and Us; and
  • other data which is considered Personal Data provided by You to Us.

Purpose of Personal Data Processing

  • to provide good service to You and fulfil Your requests;
  • to send You information about activities, marketing, or promotions organised by Us;
  • to process purchases, payments, or other transactions requested by You, including the delivery of Your orders;
  • to conduct statistical analysis for the development and improvement of Our services; and
  • for other purposes in accordance with applicable regulations.

Third Party Involvement

We may disclose and send Your Personal Data to:

  • third parties that We engage for the purpose of processing Personal Data in accordance with the purposes of processing Personal Data;
  • third parties who are service providers who cooperate with Us in providing or offering products or services; and
  • Our affiliates.

Access and Correction

If You wish to:

  • submit an access request to access a copy of Your Personal Data that We store, or
  • request correction to correct or update the Personal Data of Users that We store,

Retention and Deletion

We will cease to retain or process Your Personal Data, or delete Your Personal Data, as soon as it is reasonable to assume that such retention no longer serves the purposes for which the Personal Data was collected, and is no longer necessary for legal or business purposes in accordance with applicable provisions.

Transfer

The Personal Data We collect may be transferred out of Indonesia to be hosted on servers of our third-party data processors. In the event of a transfer of Personal Data abroad, We will take steps to ensure that Your Personal Data continues to receive a standard of protection that is at least comparable to the applicable personal data protection provisions in Indonesia.

Changes and Security

We may update this Policy from time to time. If there are changes to the way We treat Your Personal Data or to the Privacy Policy that require Your consent under applicable regulations, We will provide notification to You via email or announcement at the Pharmacy within seven (7) days prior to the change. If You continue to transact with Us, since the change in Policy becomes effective, You are deemed to have read, acknowledged and agreed to the updated Policy.

We have established and implemented security practices to protect Your Personal Data from unauthorized access, collection, use, distribution, duplication, modification, deletion or similar risks, consistent with industry standards, including implementing authentication restrictions and Open SSL encryption on APIs, among others. Access to Your data is password protected. We regularly monitor and maintain Our security standards up to date.

Personal Data Breach and Notification

In the event of a data breach, We will notify You as soon as possible (no later than 3 x 24 hours) in accordance with applicable regulations.

Liability

We are not responsible for any losses suffered by You or a third party if such losses are caused by force majeure, errors, and/or negligence on Your part.
